It is estimated that four million surgeries were performed in Japan in 2010. With one of the lowest per capita healthcare costs in the world, Japan is amongst the few Asian countries with a universal healthcare reimbursement system. Under the compulsory national health insurance program, Japanese citizens enjoy affordable and prompt access to healthcare; however Japan’s population is aging faster than almost all other major countries in the world. Currently, 21% of the Japanese population is aged over 65 and this is expected to increase to 40% of the population by 2035, which will cause a very large strain on the country’s healthcare system. Financial strain coupled with a rigid financial system has led to inadequate hospitals services and this is reflected strongly in emergency care, where in some cases, patients have been turned away.

Illnesses related to aging contribute to a large proportion of the surgeries performed in Japan, especially eye related surgeries. Cataract surgeries, for example, have one of the largest volumes of surgical procedures. Compared to other developed nations, Japan sees a high incidence of cataract removal surgeries. Other areas of surgery are proportionally lower. The Japanese diet and lifestyle significantly reduces the risk of heart disease, leading to relatively lower volumes of heart bypass surgery in the country. Bone health amongst the Japanese is also relatively good, leading to higher bone density for the aged Japanese, who generally see a lower incidence of arthroplasty surgeries.

Japan’s extensive usage of highly advanced medical technology, together with a comprehensive national health insurance scheme, underscores the country's importance to surgical device manufacturers and will continue to do so.
